Lasata Campus is currently hiring
a full time Social Worker. You must be licensed in the State of Wisconsin. Job Summary Starting hourly compensation varies based on experience.
The primary purpose of this position is to determine the need for and provide social services to residents and family in the form of financial, emotional, personal and psychosocial support through counseling, consultation and referral as needed while safeguarding the rights and confidentiality of residents in accordance with state, federal, local and facility guidelines and regulations under the guidance of the Director of Social Service. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or a logical assignment to the position.
Essential Functions And Responsibilities 45% Provide initial and ongoing assessment of resident’s social service needs including psychosocial, personal, emotional and financial in order to promote the highest level of functioning while upholding their individual rights and providing education as to their responsibilities (i.e. advance directives, care planning).
15% Assist in developing a comprehensive Interdisciplinary written Plan of Care for each resident that identifies the problems/needs of the resident and goals to be accomplished based on evaluation and assessment through the MDS system. Care plans are to be reviewed at least quarterly and updated/revised as needed due to changes in the status of the resident.
20% Make referrals to outside agencies or consultants for financial, psychosocial, discharge planning, transportation and any specialized service to meet the needs of residents (i.e. Medicare & Medicaid information, alternative living arrangements and services available in the community). A resource file will be kept in the Social Service office and updated as needed to make information available to residents, family and responsible parties.
10% Work with interdisciplinary team on a regular basis by participating in various meetings to identify and correct problems and provide improvement of services to residents, family and the facility (i.e. Nutrition at Risk, I-Team, Medicare meetings, staff meetings, department meetings as scheduled or directed by the supervisor or administration).
5% Participate in Inservice training and educational seminars to remain current on policies and procedures and to meet the requirements as a licensed Social Worker.
5% Make departmental adjustments as needed to assist the Director of Social Service in development of or adjustment to policies/procedures, assigned duties in the admission process, and in supporting and assisting co-workers in the completion of duties in a timely fashion.
Minimum Education Qualifications Education and/or Experience Requirements:
- One to two (1-2) years’ experience as a Social Worker in a skilled nursing facility or equivalent setting
- Must demonstrate the ability to work professionally with short- and long-term residents and their families in a long-term care setting.
Licenses, Certifications, and Other Requirements:
- Bachelor’s degree in social work, sociology or related field from an accredited College or University
- Currently certified or licensed as a Social Worker in the State of Wisconsin
- Must be able to demonstrate a licensure free from disciplinary action
Minimum Knowledge, Skills, and Abilities Qualifications
In order to perform the functions and responsibilities of the position (listed above) the following knowledge, skills, and abilities are essential. - Follow the directives of the Supervisor
- The ability to comply with facility, county, state and federal regulations and certification standards
- The ability to maintain accurate and timely records of residents in a confidential manner
- The ability and willingness to work harmoniously with co-workers, other departments and personnel
- Develop written care plans, complete MDS and evaluate resident needs.
- Must have the knowledge of referral sources available and the ability to access such resources
- Must have the knowledge and skills to accurately assess resident psychosocial, financial and personal needs so as to make appropriate referrals and intervene as needed

- Must be willing to perform additional job duties/tasks as described and assigned by the Supervisor or Administration
- Must be willing and able to assist co-workers in completing job duties
- Will be expected to work hours as set by the supervisor including occasional weekend and holidays
- May be subject to potentially hostile and emotionally upset residents, family members, visitors
